해부학적 특징
- femur의 neck 바깥쪽으로 이어지는 돌기 부분은 폭이 넓고 약간 앞뒤로 눌린 형태를 취하는데 그 중 위쪽 부분을 큰돌기[ great trochanter]라 부른다.
- 돌기 부분의 아래 뒤쪽에 위치하는 융기를 작은 돌기[lesser trochanter]라 부른다.
- 큰돌기에는 gluteus medius, gluteus minimus, piriformis가 insertion한다.
- 작은돌기에는 ilipsoas가 insertion한다.
Advice
- great trochanter는 다리길이 측정 시 경계지표가 된다.
- cane의 길이는 일반적으로 great trochanter의 높이에 맞춘다.
- Roser Nelaton's Line : Hip joint 45°flexion 위치에서 ischial tubercle과 ASIS를 연결한 선 (그 선상에 great trochanter가 위치한다.)
- great trochanter가 Roser Nelaton's Line보다 위쪽에서 촉진 될 때 큰돌기 고위를 판단하며, 선천성고관절 탈구의 지표가 된다.
- gluteus medius 등의 강력한 수축은 great trochanter의 열리골절을 일으킬 수 있다.
